Employment-Service and Support Admin


Cordinates intake and eligibility for Board services; assess service needs; develop Individual Service Plan for all individuals on assigned caseload; assists individuals and families in selecting qualified provider of services; monitoring of the implementation of service plans; performs Quality assurance reviews and completes reports; review unusual incidents and major unusual incidents for patterns and trends; perform 24 hour crisis intervention servics as scheduled and carry cell phone during designated rotation; completes services and support documentation and training in a timely manner, etc.

Minumum of Associate degree in Social Work or related field.  Must possess or be eligible for Ohio Department of DD Certification for Service and Support Administration/Case Management and maintain certification, valid Ohio driver's lilcense and satisfactory BCII check.
